Problem solved. I was under the assumption that InDesign uses the AppleRGB
profile for color conversions when CMS is turned off. Two Adobe documents
that I found claim that is the case.

It turns out that Indesign CS & CS2 (not sure about v2) in fact don't use
AppleRGB when the CMS is Off. Instead they use Adobe RGB (1998). Their docs
are wrong apparently.
