I've replaced the referenced PDF with a new one, this one includes my second
test which I mentioned...using a HUTCH target. The third image in the PDF
shows the ProfileMaker screen capture, selecting the correct corresponding
reference file and image, and then the 4th image in the PDF shows how a scan
looks after being the assigned that HUTCH profile. It's perfect. So what's
with the Kodak targets? I am really baffled.

The scanned image appears vertically offset in places. As you state
this is repeating with different targets I would suspect a scanner
positioning malfunction.
To check my hypothesis, try turning that target by 90 degrees and rescanning
...
