I've checked your files and I have to say that you have found a bug!
You could consider reporting it here :

But I suspect this has to do with the placed TIFF images. I experimented
re-importing them as PSDs but the bug was still there. I tried a few other
things as well but, same results. Finally, I tried replacing these TIFF
images by some other color images I happened to have lying around on my
desktop and this time, the output was correct. So, it's very possible that
these TIFF images have become corrupted somehow, Martin? Perhaps recreating
them fresh would help? But these images are not "clean", in my view. Did
this document started as an IDCS3 document in the first place or did it
start as CS, CS1 or CS2 document? Was it a carry over from QuarkXpress? Many
things can affect its performance.
