If I might add something here to this list, in recent testing I have
found the output is also affected by the monitor profile when print as
bitmap is not selected. It will be real interesting to find out where
this (monitor profile) gets enters into the print stream. And I wonder
is the monitor profile issue the whole source of this problem. Can we
say double profiling?

rather than rasterizing internally, and depends on the OS to perform
rasterizing. InDesign CS3, again by default, is submitting
PostScript data, and appears to be depending on some aspects of
PostScript color management that are getting mangled when it hits
the OS. It's unclear to me if the problem is with the PostScript and
additional metadata generated by InDesign CS3, or if it's with the OS.
In any event, the work around is to go into the Advanced section of
the InDesign CS3 print dialog, and check at the top the "Print as
Bitmap" option. That is supposed to institute the legacy printing
behavior found in IDCS2, and should resolve your problem without
having to create an application specific profile.
