You can use XRite's MeasureTool to do exactly what you describe.

Thanks everyone, I was able to make MeasureTool work in Spot Color mode. I have MonacoProfiler and of course i1Match but neither one offers a spot color reading mode.

Suddenly everyone seems to be using paint swatches instead of Pantone color guides. This works great to be able to read the supplied color chip and simply plug in that number.
