I thought I'd better post a follow-up on this issue.
I've been looking over the profiles that John kindly sent over and have yet to find what might be the underlying problem.
I was thinking it might have to do with encoding triggering a bug in the OS or driver but I'm not sure yet.
I'll keep poking at it...
