It depends on what LUT Tom meant. For speed of calculations (especially for preview purposes) tone curve is often converted to a LUT in the CMM. But in case Tom meant LUT in the profile - yes, it looks like a different issue.

Yes, and that includes icc4 monitor profiles.

Not to the same extent. In 10.6.6 the difference in presentation is tiny.
