no sound from DefaultOutputUnit

I've compiled and run the example app, DefaultOutputUnit,
but I get no 440 tone playback.

I've recently updated to the newly released API. The previous
DefaultOutput example DOES play the 440 tone; but the
current version doesn't.

The computer is plugged in and the speakers are on ;)

Any suggestions? The diagnostic output follows.
