crash in HALPlugIn::iSight::Frame::StreamHandler

my Cocoa app which installs inputIOProc sometimes (very rarely, like
one out of ten quits) crashes on quit. AudioDeviceStop() and
AudioDeviceRemoveIOProc() return noErr before the app crashes. what am
i doing wrong?
