Like you, I searched and ended up making one myself, which I posted here a while back: http://www.fourwayrecords.com/sw/ (click on AudioUnitMusicDeviceTemplates.sit - it's a Carbon view template)
I have not updated it since Tiger, and it's fairly useless in that it doesn't render any audio, but it should at least build and pass inspection on Panther.
My intent was to make a new one that generates a simple sine wave in response to MIDI note messages. That has turned into a full-blown synth project, which I'm fairly distracted with at the moment. :-) If and when I get around to making the new template, I'll be sure to post the info here.
