I know that there used to be a library that David Cope (at ucsc) owned
for doing this in os 9.
He was thinking of porting this library the last time I talked to him
(2000 or 2001) to os x.

I don't think his library was anything close to real-time though.

This is a second posting.
Quite few Lisp users are in need of RT sending and receiving MIDI
messages to/from MCL Lisp using core audio. Did anyone make a start
with this, who could give us a hand?
Thanks very much,
Peter
