Hi Phil,
Unfortunately, no, you have not overlooked some great document that
explains it all. The docs and sample apps that you and Bjorn suggest
are very good starting points.
In the relatively near future, the doc situation will improve. We'll
publish the first version of Audio Unit Programming Guide around the
time of WWDC this year. It focuses on effect units. The subsequent
version of the document will be expanded and will describe how to
write an instrument unit.

documentation for Audio Unit effects is on your system, not online:

Maybe they've got something about AU instruments, too. I'm note
