AudioUnitInitialize returning -5

Got a weird issue in Rax 2.0.... a user is loading up the default rack and when a DLSMusicDevice is AudioUnitInitialize()'d it's returning an OSStatus of -5 ....

Two questions:

What does -5 mean?

And where's the definitive list of Component error messages?
