The sample trace doesn't show anything odd. In fact, it shows the HAL
responding to the notification from IOKit that the device was plugged
in. So there's one possible culprit ruled out.
Have you seen this happen on any other machines? Does this happen
with any piece of hardware or just the one that is broken?

That's pretty weird. Have you sampled AMS to see if there are any
hung threads? The description makes it sound like the AMS process
isn't getting notified that the device has been plugged/unplugged.
