This is only observed on one machine - a G5 iMac, running 10.4.6. The
affected iMac was connected once to a faulty device. Ever since, any
Firewire or USB audio unit that I connect has this problem. I've tried
deleting

/Library/Preferences/com.apple.audio.DeviceSettings.plist

(even in single user mode), but to no avail. I also tried logging in with a
different user account, but the effect was the same. My first guess was that
somewhere there's a broken cache file arising from the original device
connect, but I've not been able to pinpoint it.
