There are no device caches like what you are thinking about. /Library/ Preferences/com.apple.audio.DeviceSettings.plist merely stores device settings that the HAL restores when the device is plugged in.

In some ways, this problem sounds vaguely like a damaged driver install or something like that. Have you tried a clean install of the OS followed by a clean install of all the relevant drivers?

That's pretty weird. Have you sampled AMS to see if there are any hung threads? The description makes it sound like the AMS process isn't getting notified that the device has been plugged/unplugged.
