Very Loud Noize

Hi all,

I have a serious problem but it is not well researched, probably something is wrong in my code but I do not know where to start. In short I have the next graph:

DefaultOutputUnit <-- AUMixer <--*a lot of input sources that are added and removed when graph is running

I add the source to the mixer to start and remove it from to stop. But sometimes (it is very hard to reproduce) I hear very-very loud noise, it's volume does not depend on volume of the device and is reproducible even when the device is muted.
