My point and my experience is that when you start to deviate from the known float/stereo/44.1 convention, do so one baby step at a time, and test each time you make a change. Do this no matter what the Apple doc says; it's a "try it and see" process. In the earlier posts in this thread you will see that Daniel got stuck because he just assumed he could get 16-bit integer samples on the iPhone. I showed him how I get floats, and now he can see if the baby step to get integers works (and if not generate them himself). I'd advise the same step-by-step for unusual sample rates, mono, etc. When you are dealing with a black box with 5-6 variables, only turn one knob at a time.
