Darren,
A while back, Pete Yandell's SimpleSynth
(http://notahat.com/simplesynth) got me started at implementing my own
Midi keyboard. It uses the default synth audio unit and requires that
you provide the keyboard code (it's just a synth), but it's free (MIT)
and it's a start.
I have since moved to writing my own cross-platform solution, but it
was cool seeing how to do a basic virtual keyoard on OS-X.
