I have discovered that our app is crashing when we remove an IOProc because of an audio plugin called Hear from a company called JoeSoft.  From what I can ascertain, this crashes your app if you do not stop your devices (we open two) in the reverse order you started them up.  But after removing and reinstalling the Hear plugin, the problem has gone away (on my machine) so I cannot investigate further.  Also, Hear.app crashes on startup on my machine (10.5.8 Intel), so I have given up.  There is a 100% correlation between customers who have this problem and Hear being installed, and vice-versa.  We have complained to the vendors. 
 
Sorry if I got a bit stroppy with you Jeff, but that documentation could definitely use some work.
