I'm not sure of this, but have a look in:
~/Library/Application\ Support/iPhone\ Simulator/<<version>>/Library/Preferences/
(where <<version>> is 4.0, 4.1, or 4.2, whichever simulator you've been running)
Do you see anything with "audio" in the name? If so, please set it aside and see if that clears things up.
