kAudioSessionProperty_ServerDied

Hey CoreAudio,

I'm seeing and monitoring the message, kAudioSessionProperty_ServerDied.

I see it happen will some regularity on iPod Touch 2nd devices running
OS 3.1.3 and 4.0.2.

I've got two questions:
1) In what conditions is this caused? Crossing the Audio Beams for
example. Something I'm doing is causing it, but I don't know what it
is.
2) If it happens, what can I do to get everything back? I reallocate
my audio session, audio units, session category, but no audio flows.
