Currently I'm using a variant of CAPlayThrough to route audio from the Bluetooth Audio driver to the selected mic and speakers. I've seen previous mention of a VoiceProcessingIO Audio Unit for iOS, but my Google search skills have yet to turn up anything I can deploy.
