Couldn't that be handled by kAudioUnitEvent_PropertyChange on the kMusicDeviceProperty_MIDIXMLNames property so a host would be notified of such changes?

I did look at it, but as far as I could see, it is file-based, and
therefore the list becomes out of date the second that the user edits
a voice name in the virtual instrument.
