Headphone-only audio bug on iPad 2

Hi folks. I'm getting complaints from iPad 2 users that my app only
makes sound through the headphones. I'm unable to reproduce the bug on
my own iPad 2.  Any ideas of what sort of conditions would lead to
headphone-only audio on some but not all iPad 2's? All of my audio
passes through the RemotIO audio unit, for what that's worth.
