My assumption will be that modifying the call app is not an option, since
otherwise this would be trivial of course.
The only approach that I can imagine would be to use Jack (or something
similar) to reroute the mixed sound into the caller app. In any case, this will
require some form of user interaction: the user has to select the Jack server
as the audio input instead of the microphone, which is probably confusing
for non-power users. Regards,
