However I did re-read a number of related documents last night in the
hopes that I'd missed something before, but unfortunately I hadn't. :-)

A few more hours with google gives some more clues that a PAM module
called pam-tally could work (that's what we use on our Linux systems
to meet this requirement), but I am not able to locate a Mac version
of this module (and not being a Mac developer, I expect that it would
take me quite some effort to build it or port it). If anyone has a
version that would work for 10.3.9, I'd be grateful to get a copy.
