I've never once seen a vendor
include cage nuts since it's something you generally provide with your rack. You
can purchase small bags of them from most suppliers which sell racks and the
like.
In the past I've seen a lot
of people complain about square holes vs. racks with tapped holes, I
can see where the complaints come from but in my experience racks that give you
the option of hanging the equipment or putting cage nuts in are a lot more
flexible. It sucks to toss a rack away when you strip the holes and have no way
to fix them minus trying to tap them again. It's much easier to pop out one cage
nut and throw in another.
For all of those killing their fingers, I
used to maintain a data center cage with 16 racks so I spent a lot of time
popping cage nuts in and out however I never had any issues chewing up my
fingers. A small flat head screw driver for popping them in and either that or
pliers for compressing the pins to pop them out will save your
hands.

My group uses Dell PoweEdge racks (square holes)
extensively since (duh) we
have lots of Dell rackmounts. Mounting a
Dell RapidRail system takes
literally about three minutes, usually including
opening the box.
And then there is the "legacy" equipment, including
Xservs, that hasn't
caught on to the "square holes are wonderful"
phenomenon. Put in 4-8 cage
nuts, screw in the rail sides and/or
plates, insert the rackmount. Takes
about 15 minutes per server.
Frequently sped up by the blood sacrifices
previously
mentioned...
