Shawn,
Jerry mentioned crashing of the Keychain, which is a similar problem that I'm experiencing (I can access my CAC, I just can't set identity preferences to actually use it). Are you strictly collecting data for the "my card reader doesn't work at all" situations, or for all CAC functionality, or lack of?
Is there a list of known good configurations that work? I've seen a few messages on this list contradicting each other about working configurations.

The issue that folks initially raised was related to a failure to be able to access the CAC at all on Mac OS X 10.5.x with their particular Smart Card and Smart Card Reader. Note that if the Smart Card does not appear in the Keychain Access utility as a dynamic keychain, the Reader/Card combination is not being recognized by the system.
From everyone I have received info from, it is isolated to the combination of the following Readers and Card:

There is no work-a-round for this particular issue other than using a different Reader until this is fixed -- which is not possible for many people on this list.
