David,
To reiterate, the issue is the combination of that reader and the
Oberthur card. i would assume then that your CAC is not an Oberthur
ID One v5.2. If it is not that explains why things work fine for
you. If your card is the Oberthur ID One v5.2 card, please provide me
directly with more information on your system.

The issue that folks initially raised was related to a failure to
be able to access the CAC at all on Mac OS X 10.5.x with their
particular Smart Card and Smart Card Reader. Note that if the
Smart Card does not appear in the Keychain Access utility as a
dynamic keychain, the Reader/Card combination is not being
recognized by the system.
From everyone I have received info from, it is isolated to the
combination of the following Readers and Card:
