As an added note, I tested -- with the same result -- using a new browser now in Beta: Shiira. This browser is built on the same platform as Safari: Web Kit.
So the problem, it appears, is not with Safari per se; but rather with its engine- Web Kit.
FYI, Shiira has some interesting features in that it has "tabbed" browsing but the "tabs" show up as real-time thumb-nail images of the page -- i.e. they get updated and the thumb-nail shows the update even if they are not the forepage. It has some promise.

Just to follow up on this one. I also have the same issue on my MBP.
I tried deleting Safari Cache entries and Safari plist entries, but still no icons showing in the AKO 'Files' mode.
Obviously our setup is different than Paul Villano, who has it working on his MacBook...
