The home button can be disabled via software in iOS 6. I recall info from WWDC on that. It is implemented by the app developer, not a system feature.

There are some really nice kiosk products that cover the home button with the stand hardware that would be awesome for us in clinical areas like:

Which implements the kiosk mode on the device. More for actual kiosk than toddler.
