It has to do with Outlooks Pathetic caching and monolithic database
I can usually resolve by changing the directory server to a different server (tools - accounts - advanced), saving, quitting outlook, and relaunching
Many times - the setting will revert back to what it was before (depending on your exchange setup), but something about that triggers the client to refresh lookups
Sounds like voodoo - I know
