That was my experience as well.
Hopefully, others will read this post and avoid having to spend to
much time trying to figure it out on their own.
I suppose this is technically a but that we should all report.
Otherwise, unless someone from Apple actually monitors this list, it
won't get noticed and fixed.

I had the same problem. Since I'm a novice, I spent lots of time
going down rabbit trails. My final posting to the thread of the
above subject was an effort to explain what seemed to be the
conditions that caused it for me.
For me, it seems that Leopard's PackageMaker fails to work for
custom icons for any folder at the root of the source for the
package. The "Include root in package" option gets the root
folder, but not the root folder's icon. But folders nested within
that root did seem to preserve custom icons.
At least this was my experience.
