These are often home grown systems, from what I have seen int he wild, typically based on FileMaker Databases. For example, you can download, for free, this FileMaker Application "lending Library" that handles check in and check out services:

You also might get what you are looking for out of a asset management system such as Web Help Desk:

WHD will certainly not be the scan with laser, swipe ID card system that we often see developed in-house.
