FYI, the server was running CentOS 5.3 so we upgraded everything, including the kernel, to CentOS 5.8. Reboot (to activate the new kernel) and the problem went away. So I don't know what was the problem, but looks like it was a bug somewhere that was affecting system resources or something ("ulimit" is still set to 1024 after the upgrade).
