Brian,
I tried out the CodeTek demo, but this is not quite what I'm looking
for--there are several problems with this "solution":
1) Doesn't work with Apple's X11 (maybe it does, but it didn't for me)
2) Auto-raises the focused window.
This is a terrible way to hack the 'cursor focus' behavior because it
doesn't actually add any new functionality--I might as well just click
the window. What I want, at least, is a way for keyboard events to be
delievered to a background window without having to bring it to the
foreground.
3) I have to pay $30 -- Apple should expose this to the end user without
requiring them to purchase some 3rd party utility.

Already supported in Apple X11 server for X apps, and in OS X as a whole
you can enable it with CodeTek's Virtual Desktop.
Right. What you asked for, isn't really what you want. Like me you want more that simply
'focus follows mouse' - you want the whole deal, ie focus-follows-mouse, which probably means
menus in windows, plus no-auto-raise :) All as config options, of course, so that people
who like it the way it is, don't have to change.
There are many windowing systems allow the above, while also allowing it to be the way Apple
has it. I am suprised Apple is taking so long to 'come round'.
BTW, IIRC, DEC's VMS window manager, although it was X, still implemented 'click to focus' and
'auto raise' as default behaviour, so some people actually choose that way even when they
have the option.
