This is a known problem and has already been fixed for the next public
release (don't ask me when that will be :-)

Yes, I've noticed missing icons, but it's not reproducible in the way
that you say. Seems to be more related to the order in which apps are
iconified. For example, I just tried minimizing and reopening an
xterm, and I was able to do it 10 times without failure. But when I
had two apps (xterm and xv) being minimized and reopened, the error
happened then.
