That's because Aqua can't do focus-follows-mouse (and almost certainly
never will). Your FFM setting only works if they X server is the
currently focussed application. I agree this is less than ideal, but
I don't think there's anything that can be done to work around this at
the moment.

Works for me, but then I use quartz-wm as the window manager rather than
fvwm2.
