I've seen it happen on rare occasions for versions of OSX since at least 10.2 (can't recall about 10.0/10.1) -both X11 terminals and Terminal.app.
Basically, what happens is that the shell does not get started correctly, thus ending the xterm (or immediately ending the shell in the Terminal window).
My own suspicions are that it may be to do with the user lookup system under OSX -it may occasionally not quite work correctly (I haven't investigated -it's such a minor inconvenience as I can start up another terminal within a couple of seconds).
My reasons for this thinking relate to my experiences on an iMac G5 that I use occasionally. It used to be on a network with user accounts provided from a 10.4 server, but it was moved elsewhere. Once moved, I found I could not log in as a local user I had subsequently created. I eventually got around this by removing some old user info stuff I found cached in the NetInfo database (in mcx_cache, IIRC).
However, I then found it was frequently taking a very long time to come up with a password box from the screensaver (spinning cursor for two or three mins before the box appeared). Of more relevance to this topic, I also found that it was often taking a very long time to start up a terminal window (of any description) -the window would appear, and then it would sit for a couple of minutes before continuing with output from my shell init scripts and on to the prompt.
I did manage to fix these problems (both at the same time, suggesting a link -though I'm not sure how I did it), but it does seem clear that something has to happen as far as user lookup goes (or some other similar info) before it can properly start a shell (interactive? for a terminal window of some sort? -never seen it via ssh... but what would be different for a terminal window?)
So, there you have my reasoning (certainly not complete, but suggestive) regarding these immediately-terminating shells.
If others know more about this kind of thing, perhaps they could investigate?
