I see the same problem with windows that are either minimized or
withdrawn at the time that I plug an external monitor into my laptop.
X11 moves all its windows to the new screen ... and at the last location
of any minimized/withdrawn window, there's a white rectangle. The only
way to get rid of it seems to be to remap the window or exit the
responsible application.
I think I've only seen this with remote emacs and tcl applications, but
that might just be an artifact of my usage patterns.
