If I start an xterm from Terminal.app, there is no problem with that
xterm. RE Bernard's observation, yes that (the xterm staying there
but not activatable) happens to me too if I have a process running in
that initial xterm.

Hmm... I wonder if this has something to do with the fd addition
code... I don't use app_to_run, so that's why I hadn't noticed it.
I didn't realize that's what you meant by "first xterm" ... does it
happen if you run 'xterm' from within Terminal.app to start X11 and
treat that as the "first xterm"?
The first xterm stays there, but I can no longer use it: it does not
become active when I click it with the mouse.
I get this message in Terminal.app when closing the second xterm
Xlib: unexpected async reply (sequence 0x6e00)!
If I open the second xterm by typing the command in the first
xterm, and the close it, the first remains...
I use bash & have no .xinitrc in my $HOME
