I have the same problem, too, using the same versions of everything running on a
new Mac Mini.
If I select X11 a second time using command-tab, the windows do all come to the
front. If I use the dock to switch to X11, it works the same way. The first
click activates X11; the second brings all its windows forward.

I tried installing the latest version of xquartz from sourceforge (2.4.0). I
am running 10.5.8
I'm a bit worried I may have messed up my xquartz installation - so
yesterday I did a clean reinstall of Leopard (backing up from time machine).
I also followed the FAQ for de-installing X11 and then reinstalled 2.4.0.
However, the problem persists.
