Then why don't you operate the MacBook with the lid closed? That

19" LCD will be your main and only screen.
It is because it heats up more rapidly (fan) and after a while the
screen
gets dirty with the heated dirt.
Anyway, the X server should honor the -xinerama command-line
argument, as it
is displayed when I launch manually the server with wrong arguments :
It should honor the flag, but -xinerama disables the extension, not
the use of multiple monitors. Note that this is not really supported,
though.
You should disable that monitor if you don't want to use it. You'll
run into problem in general that are not specific to X11.
