Cmd-` sends focus to the next application window... Try tapping the Cmd keys.
I usually see this with VNC, where it's arguably a protocol bug, but have
seen it with X11 once or twice as well. No idea if it's fixable, although
if you have a 100% reliable way to trigger it then maybe it'll be possible.
