If you're seeing a problem where an where the wrong version of a file is being built, then we really want to hear more details. And the best place for that is <http://developer.apple.com/bugreporter>. From your description, it seems most likely that this is related to "Predictive Compilation". You can try turning that off (in the "Build" preferences) to see if that helps, but please do file a bug.
Indeed, I had predictive compilation turned on. My apologies for jumping to conclusions so quickly - I could have sworn that I had predictive compilation turned off, and I saw that extra thread appear and disappear after every save.
I'm afraid I can't give you a case that reproduces every time, I did have several instances where I got compiler errors from outdated file versions. I will remember to file a bug report next time I encounter that problem.
