We need the xcode-gdb log file. Set the user default
'PBXGDBDebuggerLogToFile YES' to log Xcode <-> GDB communication to /
var/tmp/folders.<UID>/Temporary Items/XCGDB-<process>-<pid>
TerminalPrompt> defaults write com.apple.Xcode
PBXGDBDebuggerLogToFile YES
Then reproduce the problem, file a bug report and attach the log.

1. Why does this happen? The file is in the right place, it appears
to be intact and is readable for everyone.
2. Does Xcode need to tell me about it in a modal sheet that it's
showing me at every single breakpoint? There are less intrusive
ways of telling me that a plugin couldn't be loaded, and telling me
once would be sufficient.
