I have been very happy with doxygen. It integrates well with Xcode (there are a number of tutorials online and in print that discuss how to set up a doxygen target for Xcode, google "doxygen AND xcode") and I've found its support for C and C++ to satisfy my needs. I don't do much Obj-C programming, but the doxygen site says that it handles it also. Dimitri, the author, was responsive when I had questions for him (a couple of years ago).
