I think the "excuse" is likely the usual one: engineering triage. Here's a hypothetical conversation at the Xcode development department:

It's my position that if you still have major problems like this after five years the engineering team is either understaffed, poorly staffed, or badly managed. My assumption is that it's understaffed. And I'm sure there are bean counters at work in this who think that since we're programmers we don't mind dealing with usability issue. In fact, we like to suffer. To them understaffed == adequately staffed. ;-)
