Ensure that changes are getting saved properly in the project file.
The right way to remove a file from the project is to select the file within the "Groups & Files" section of the project and press delete. It would ask you two options-
1. Delete Refrences & files
2. Delete References
either way the file will be removed from the project completely.
Adding Icon:
Then right click on Resources > Add > Existing Files > (select the new icon file) > Add it relative to project.
Change the CFBundleIconFile in info.plist to NewIcon.icns
Clean all targets. Rebuild.

I do not have the "Textpad" application on my Mac, so I use TextWrangler
instead. There was three occurrences of the string "
oldIcon.icns"
in the project.pbxproj file, and I replaced them all with newIcon.icns.
The problem stays the same, however.

Er, define "removed properly" (see below).

I erased it with a "rm" command inside the terminal (and made
sure with
a "find . -name oldIcon.icns"" command that it was not in the project
anymore).
Then the file appears in red in the usual Xcode interface, and I deleted
it a second
time (that is, I deleted the reference in the project).
